  • Drivers License
  • Social Security Card
  • Nursing License/State Certificate
  • Current CPR Card
  • Specialty Nurses for specific areas need ACLS,PALS,NALS,NRP
  • Physical Exam – within last 6 months
  • PPD or Chest Xray – within 6 months
  • Original 4 hour – Aids Update
    (if expired then we need current Aids 1 hour update)
  • Domestic Violence – 1 hr Update
  • Immunization Record – Proof of MMR
  • Proof of Hepatitis C Titer - Hospitals only
  • Proof of Hepatitis B Shot
  • 10 panel drug screen

Level 2 FBI screen required prior to being hired.

In-services available to new hires and current employees:

Aids 4 hour & 1 hour updates
Domestic Violence – 1 hour updates
Sitter/Companion & HHA classes
Many other CEU’s available upon request
